Application forms are available at the following locations:

• Hang Seng Bank branches (MegaBox branch is located at Shop 19, G/F)
• MegaBox Customer Service Counter (2/F and 10/F)
• Promotion standees at MegaBox
• MegaBox Card roadshows at MegaBox (please contact the MegaBox Customer Service for details of the dates and venues)

Simply complete the application form and mail or return it to any Hang Seng Bank branch with copies of your Hong Kong Identity Card and your bank statement / passbook showing your latest month’s income.
